Трехмерная оценка морфологии корневых каналов постоянных верхних первых и вторых премоляров у населения Ирака с использованием конусно-лучевой компьютерной томографии (ретроспективное исследование)

Аннотация

ВВЕДЕНИЕ. Оптимальный результат эндодонтического лечения во многом определяется качественным инструментальным препарированием и формированием системы корневых каналов, тщательной механической обработкой и эффективным протоколом окончательной дезинфекции. В связи с этим исследование сложной морфологии системы корневых каналов с использованием надежных и высокоточных методов является крайне важной задачей.

МАТЕРИАЛЫ И МЕТОДЫ. В исследование были включены 384 изображения, полученные методом конусно-лучевой компьютерной томографии, содержащие в общей сложности 620 зубов, принадлежащих пациентам из Ирака в возрасте от 15 до 45 лет. Исследуемые снимки были получены в период с 2019 по 2024 г. Верхние первые и вторые премоляры (U4 и U5) анализировали в трех плоскостях (аксиальной, коронарной и сагиттальной) с целью определения количества корней, количества корневых каналов, их конфигурации (по классификации Vertucci), а также выявления аномалий строения зубов. Полученные данные сопоставляли с полом пациентов. Для статистического анализа использовали критерий χ2. Пирсона при доверительной вероятности 95% и уровне статистической значимости p < 0,05.

РЕЗУЛЬТАТЫ. У 70% исследованных первых верхних премоляров (U4) выявлено два корня, тогда как трехкорневые и однокорневые зубы составили соответственно 1 и 29% выборки. Среди вторых верхних премоляров (U5) у 79% был обнаружен один корень, тогда как двухкорневые зубы составили 21%, а трехкорневые не встречались. Для первых верхних премоляров наиболее часто определяли два корневых канала (71%), тогда как один и три канала встречались в 27 и 1% случаев соответственно; преобладающей являлась конфигурация каналов типа V по Vertucci. Для вторых верхних премоляров в 70% случаев выявляли один корневой канал, а в 29% – два канала; наиболее распространенной была конфигурация типа I по Vertucci. Для большинства исследованных параметров установлена статистически значимая связь с типом зуба и полом пациента.

ЗАКЛЮЧЕНИЕ. В настоящем исследовании с использованием конусно-лучевой компьютерной томографии была изучена внутренняя анатомия корневых каналов постоянных верхних премоляров на выборке населения Ирака. Выявлены различия в распространенности анатомических вариантов в зависимости от пола пациентов и типа зуба. Клиницистам следует учитывать выраженное анатомическое разнообразие первых верхних премоляров, несмотря на низкую вероятность наличия более двух корней и более двух корневых каналов, а также возможность более сложного строения системы корневых каналов в средней трети корня. Эти особенности могут способствовать более качественному проведению эндодонтического лечения и, следовательно, улучшению его результатов. Вместе с тем полученные данные не позволяют рекомендовать обязательное выполнение трехмерного исследования перед проведением эндодонтического лечения данного зуба.
