Сравнительная эффективность наиболее распространенных растворов для ирригации корневых каналов

Аннотация

ВВЕДЕНИЕ. Заболевания, связанные с патологией системы корневых каналов, остаются одной из актуальных проблем современной эндодонтии вследствие сложности достижения полной элиминации микроорганизмов из корневого канала. Выбор оптимального ирригационного раствора является ключевым фактором, определяющим эффективность дезинфекции и долгосрочный прогноз эндодонтического лечения.

ЦЕЛЬ. Провести анализ современных литературных данных, посвященных эффективности наиболее широко применяемых ирригационных растворов для обработки корневых каналов, а также оценить их преимущества, ограничения и возможности рационального клинического применения.

МАТЕРИАЛЫ И МЕТОДЫ. Выполнен анализ современной научной литературы, посвященной применению наиболее распространенных ирригационных растворов в эндодонтии – гипохлорита натрия (NaOCl), хлоргексидина (CHX) и этилендиаминтетрауксусной кислоты (ЭДТА). Проведена сравнительная оценка их антимикробных свойств, способности растворять органические ткани, влияния на структуру дентина, особенностей клинического применения и эффективности при использовании в составе комбинированных протоколов ирригации.

РЕЗУЛЬТАТЫ. Анализ литературы показал, что каждый из рассматриваемых ирригационных растворов обладает собственным механизмом действия, преимуществами и ограничениями. Гипохлорит натрия остается «золотым стандартом» ирригации благодаря высокой антимикробной активности и способности растворять органические ткани. При этом снижение его концентрации в сочетании с нагреванием и физической активацией позволяет сохранить эффективность при уменьшении токсичности и отрицательного влияния на свойства дентина. Хлоргексидин характеризуется широким антимикробным спектром, низкой токсичностью и выраженным пролонгированным действием, однако не обладает достаточной тканерастворяющей способностью и требует соблюдения протокола применения для предотвращения образования потенциально токсичного осадка при контакте с NaOCl. ЭДТА эффективно удаляет смазанный слой и неорганические компоненты дентина, улучшая проницаемость дентинных канальцев и условия для последующей обтурации. Наибольшая клиническая эффективность достигается при комбинированном использовании ЭДТА и NaOCl, обеспечивающем воздействие как на органические, так и на неорганические компоненты системы корневых каналов.

ЗАКЛЮЧЕНИЕ. Ни один из современных ирригационных растворов не является универсальным для эндодонтического лечения. Наиболее обоснованным подходом представляется их рациональное комбинированное применение с учетом клинической ситуации, используемой концентрации и методов активации, что позволяет обеспечить оптимальный баланс между эффективностью антимикробной обработки, безопасностью и качеством эндодонтического лечения.
